PURPOSE NorthWest Arkansas Community College (NWACC) issues this Request for Proposal (RFP) to obtain bids for a new benefit for NWACC employees – Family Forming. The proposed date for the program to be effective April 1, 2025. Northwest Arkansas Community College currently covers Infertility testing, and once a diagnosis of Infertility has been established, NWACC covers artificial insemination and in vitro fertilization with a lifetime maximum of $15,000. Please provide a pricing proposal for fertility treatment and family building services per the guidelines below. Please also complete the attached questionnaire on Page 20-27 to the best of your ability. Lockton serves as the ongoing insurance consultant/broker for NWACC and has been retained to assist in the evaluation of the proposals for completeness and responsiveness.
