The Hawai'i County Office of Aging (HCOA) is requesting proposals from qualified applicants to provied Case Management Services. The intension of this project is to provide assistance in the form of accessto services and care coordination in circumstances where the older adult is experiencing diminshed functioning capacities, persnal conditions or other characteristics which require the provision of services by compensated service providers or family caregivers. Family Caregiver Case Management is to provide support services that address the needs of family caregivers to enable themto maintain their abilities to continue providing care.
