The Hawai‘i County Office of Aging (HCOA) is requesting proposals from qualified applicants to provide coordination of the Better Choices Better Health (BCBH) – Ke Ola Pono Chronic Disease Self-Management Program (CDSMP). The intention of this project is to continue development and implementation of the BCBH – Ke Ola Pono program throughout the County of Hawai‘i. The Better Choices Better Health (BCBH) – Ke Ola Pono is an evidence-based Chronic Disease Self-Management Program (CDSMP) developed and tested by Standford University designed to complement and enhance medical treatment and disease management. Research has shown that BCBH participants are more able to manage their symptoms and have an easier time communicating with their health care providers and loved ones about their condition(s). Currently, the Hawai‘i County Office of Aging, through it’s Healthy Aging Partnerships (HAP), is able to offer the BCBH Chronic Disease Self-Management Program, as well as the more targeted BCBH D
