Take careful note of the addendum so that proper allowance may be made. Changes to Contract Documents: Page 25-26 (Delivery Sites & Contact Information) Fountain Valley School District is adding the following delivery location to their delivery sites: Fountain Valley Food Service Warehouse 17330 Mt. Herrmann Fountain Valley, CA. 92708 Changes to Proposal Form Pricing Sheet: Page 40-68 Modifications have been made to the proposal form pricing sheet, Bidders are instructed to use and/or request the updated list* and submit with proposal documents. *See attached list in following pages. Bidder is to use the Pricing Sheet template provided on the accompanying proposal documents. a) The Pricing Sheet must accompany the completed formal proposal. b) Bidder is to complete requested information on Pricing Sheet. c) If Bidder would like to submit the Proposal Form Pricing Sheet via electronic submission, please send a request to Emily Hughes, Purchasing Supervisor at ehughes@ggusd.us to obtain an excel spreadsheet. If Bidder is opting to submit an electronic proposal, a USB drive MUST be submitted with their proposal documents. Any electronic Proposal Form Pricing Sheets that are received via email will be disregarded and will not be considered with the Bidders submitted proposal. 2. Bidder is to submit all pricing spreadsheet pages, even those without responses. 3. The Participating Districts reserves the right to reject proposals with multiple items per line item. Bidders submitting proposals with more than one item per line item may be rejected or be deemed as non-responsive. 4.
