RFP ERO DO #02-F-1-26/27 - Mobile Health Services

Description

Carroll County Health Department (CCHD) and the Frederick County Health Department (FCHD) are issuing this Request for Proposals (RFP) to provide Mobile Health services in our subregion (Carroll and Frederick Counties) in order to increase the infrastructure and improve access to care for residents in our community. The Carroll County Health Department (CCHD), in collaboration with Frederick County Health Department (FCHD), (Heretofore, combined to “CC/FCHD”) are working together as a subregion, to present this RFP to qualified organizations to design, expand, and/or implement mobile health access strategies including home-based, community-based, or place-based services for residents of Carroll and Frederick Counties. Funding is supported through the Maryland Rural Health Transformation Program (RHTP) and must align with the purposes of improving access to care, strengthening rural health infrastructure, improving healthcare quality and outcomes, and transforming the healthcare deliver
