Mail Order Incontinence and Urinary Supplies and Services RFP No. 2025HCA15 Amendment No. 04 Date Issued: 3/17/2026 To: RFP Bidders From: Jennifer Burnham, RFP Coordinator Purpose: Amend RFP to post the Interested Subcontractor List and provide a response to a question that was mistakenly excluded from the 2nd round of bidder questions and answers. This amendment hereby modifies and is attached to RFP No. 2025HCA15. All other terms, conditions, and specifications remain unchanged. The above referenced solicitation is amended as follows: 1. HCA received questions from the 2nd round of bidder questions and posted answers to those questions in Amendment #03, however, HCA mistakenly excluded the following question and response in Amendment #03. The excluded question and HCA’s response are provided below: Question: May a licensed Nurse Practitioner who also serves as the ordering practitioner for the patient's incontinence supplies perform the nursing assessment required under Section 3.5? Answer: Nursing assessments must be performed by the contractor’s licensed nursing staff in accordance with the requirements in the Scope of Work and may not be delegated to outside entities unless those entities are subcontracted by the contractor as outlined in the RFP. 2.
