Mail Order Incontinence and Urinary Supplies and Services RFP No. 2025HCA15 Amendment No. 03 Date Issued: 3/13/2026 To: RFP Bidders From: Jennifer Burnham, RFP Coordinator Purpose: To amend RFP and Post Answers to 2nd Round of Bidder Questions This amendment hereby modifies and is attached to RFP No. 2025HCA15. All other terms, conditions, and specifications remain unchanged. The above referenced solicitation is amended as follows: 1. Exhibit D – Cost Proposal is replaced in its entirety with Exhibit D–Cost Proposal(Rev) due to outdated SKU numbers being referenced for some products, and to add three hydrophilic catheter products to the Cost Proposal. All Bidders are required to ensure they are referencing and using the correct RFP documents when submitting their proposals. 2. Attach 1-WA Average Product and Client Usage is replaced in its entirety with Attach 1-WA Avg Product & Client Usage(Rev) to add three hydrophilic catheter products to the Attachment. All Bidders are required to ensure they are referencing and using the correct RFP documents when submitting their proposals. 3. RFP Section 3.10 Cost Proposal, Subsection A, Identification of Costs is amended to read as follows: i. Identify all costs in U.S. dollars including expenses to be charged for performing the services necessary to accomplish the objectives of the contract. ASB(s) will be required to collect Washington State sales and use taxes from HCA, as applicable, and for remittance of payment to the Washington State Department of Revenue (DOR). Bidders must include any expenses to which Washington State sales and use taxes apply in the Cost Proposal and include an estimated amount for such taxes (based on the current tax rate(s)). HCA understands these amounts may fluctuate as tax rates fluctuate. 4. HCA received questions from the 2nd Round of Bidder Questions.
