Mail Order Incontinence and Urinary Supplies and Services RFP No. 2025HCA15 Amendment No. 11 Date Issued: 6/10/2026 To: RFP Bidders From: Jennifer Burnham, RFP Coordinator Purpose: To amend RFP Solicitation Schedule, revise Section 3.10, Subsections A and B, replace Exhibit D, Cost Proposal, and respond to complaints. This amendment hereby modifies and is attached to RFP No. 2025HCA15. All other terms, conditions, and specifications remain unchanged. The above referenced solicitation is amended as follows: 1. RFP Section 3.10 Cost Proposal, Subsection A, Identification of Costs is amended to read as follows: A. Identification of Costs i. Identify all costs in U.S. dollars including expenses to be charged for performing the services necessary to accomplish the objectives of the contract. ASB(s) will be required to collect Washington State sales and use taxes from HCA, as applicable, and for remittance of payment to the Washington Department of Revenue (DOR). Bidders must identify any expenses to which Washington State sales and use taxes apply in the Cost Proposal and include an estimated amount for such taxes (based on the current tax rate(s)). HCA understands these amounts may fluctuate as tax rates fluctuate. 2. RFP Section 3.10 Cost Proposal, Subsection B Exhibit D, Cost Proposal is amended to read as follows: B. Exhibit D, Cost Proposal i. Solicitation Amendment In Exhibit D, Cost Proposal, there are fifty-two (52) incontinence and urinary Products HCA is procuring, and those Products are identified by Item No. (Column A), HCPC Code (Column C), Item Description (Column D), and the most common Referenced Brands/Manufacturers (Column P). Page 1 of 6 ii. Bidder must provide Unit Cost Per Item for each Item listed by completing Columns G, J, and M. Bidders must provide the Unit Price Per Item exclusive of sales and use tax. HCA will reimburse for sales and use tax in addition to Unit Price Per Item using applicable destination shipping zip codes of Product(s). Bidder will provide the estimated sales and use tax per item by completing Columns H, K, and N. iii. Bidder must provide a Total Bid Amount for each item listed by completing Columns I, L, and O. iv. The Cost Proposal’s Total Bid Amount for each item must include, but is not limited to the following: a. Cost for Bidder’s services as described in Section 1.5, Scope of Work. b.
