The Hawaii County Office of Aging (HCOA) is requesting information from qualified applicants to provide coordination of the Better Choices Better Health (BCBH) Ke Ola Pono- Chronic Disease Self-Management Program (CDSMP) throughout the County of Hawai'i. Contracts are planned to begin on or after October 1, 2026. The purpose of this Request for Information (RFI) is to assist HCOA in obtaining community input for the development of Request for Proposals (RFP) for Coordinator of the BCBH Ke Ola Pono- Chronic Disease Self-Management Program. HCOA is specifically interested in feedback and comments regarding the items listed below. We welcome additional comments and suggestions. 1.
