The Diabetes Prevention and Control Program is requesting applications to establish Regional Diabetes Coalitions to strengthen local efforts in diabetes prevention, education, and management. One lead organization will be selected in each of the four regions of the state—Northern, Central, Southern, and Delta—to oversee and guide coalition activities within their district. Each selected organization will receive up to $18,500 in funding to establish and operate the Regional Diabetes Coalition, ensuring the capacity to build strong partnerships and advance diabetes prevention and control efforts. These organizations will convene healthcare providers, community-based organizations, public health professionals, and other key stakeholders to collaborate on addressing the growing burden of diabetes in their respective communities.To support this initiative, the Diabetes Prevention and Control Program will partner with each Regional Diabetes Coalition to host quarterly meetings that facilitate resource sharing, foster cross-sector partnerships, and coordinate regional strategies aimed at reducing diabetes-related health disparities and improving health outcomes statewide.Applicants must be located in the state of Mississippi in the following counties: Holmes, Leflore, Wilkinson, Sharkey, Issaquena, Tunica, Coahoma, Claiborne, Noxubee, Humphreys, Adams, Bolivar, Sunflower, Jefferson Davis, Pike, and Clay.
