The Diabetes Prevention and Control Program (DPCP) is seeking qualified applicants for the 2025–2026 Diabetes Prevention Program (DPP) subawards. This Request for Proposals (RFP) invites eligible organizations to apply for funding to establish or expand CDC-recognized DPP lifestyle change programs. The goal is to prevent or delay the onset of type 2 diabetes among adults with prediabetes through structured, evidence-based interventions focused on healthy eating, physical activity, and weight loss.Through this funding opportunity, up to four subgrantees will each be eligible to receive awards of up to $15,000 for two years. The funding period will run from August 15, 2025, to June 29, 2027. Awarded organizations will be expected to meet key deliverables, including enrolling eligible participants, offering the full year-long CDC-approved curriculum, collecting and reporting required program data, and engaging in outreach and retention efforts to ensure program success.Applicants must be located in the state of Mississippi in the following counties: Holmes, Leflore, Wilkinson, Sharkey, Issaquena, Tunica, Coahoma, Claiborne, Noxubee, Humphreys, Adams, Bolivar, Sunflower, Jefferson Davis, Pike, and Clay.
