The Diabetes Prevention and Control Program is offering a funding opportunity of up to $18,500 each for three pharmacy associations interested in establishing either a Diabetes Self-Management Education and Support (DSMES) program or a Diabetes Prevention Program (DPP).This RFA supports the development and implementation of nationally recognized DSMES or DPP programs designed to improve diabetes prevention, education, and health outcomes—particularly in underserved and high-risk communities. Funded services must be delivered between August 15, 2025, and June 29, 2027.Eligible applicants located in Mississippi that are committed to achieving accreditation or recognition (e.g., through ADCES, ADA, or CDC) for DSMES or DPP programs.
