The Diabetes Prevention and Control Program (DPCP) is seeking qualified applicants for the 2025–2027 DSMES (Diabetes Self-Management Education and Support) subawards. This RFA invites eligible healthcare providers and organizations to apply for funding to establish or expand ADCES-accredited DSMES services. The goal is to enhance access to quality diabetes education and empower individuals with diabetes to effectively manage their condition.Through this funding opportunity, up to four subgrantees will each be eligible to receive awards of up to $18,500. The funding period will span from August 15, 2025, to June 29, 2027. Awarded organizations will be expected to meet key deliverables, including applying for recognition or accreditation, delivering culturally appropriate DSMES services, increasing participant engagement, and collaborating with community partners to promote program sustainability.Priority consideration may be given to applicants serving rural, underserved, or high-burden communities across Mississippi. Proposals should clearly outline the implementation strategy, staffing plans, and measurable objectives aligned with national standards for DSMES. This initiative supports the state’s ongoing efforts to reduce the burden of diabetes and improve health outcomes through education, empowerment, and equitable access to care.Applicants must be located in the state of Mississippi in the following counties: Holmes, Leflore, Wilkinson, Sharkey, Issaquena, Tunica, Coahoma, Claiborne, Noxubee, Humphreys, Adams, Bolivar, Sunflower, Jefferson Davis, Pike, Clay.
