The Pasadena Outreach Response Team (PORT) is a multidisciplinary, field-based program designed to engage individuals experiencing homelessness who face complex barriers related to mental health conditions, substance use, chronic medical needs, and justice system involvement. PORT prioritizes low-barrier, trauma-informed, and person-centered engagement strategies to stabilize participants and connect them to housing, healthcare, and supportive services.To strengthen participant engagement and continuity of care, the City of Pasadena Public Health Department (PPHD) seeks to contract with a qualified nonprofit agency to provide peer-led outreach and engagement services.
