On “Part 1” of the bid form, shipping/delivery cost of the items should not be included in the price of the items (Items 1-7). All delivery/shipping costs should be included as a lump sum total in Item 8. 2. Bids will not be accepted via fax or email. Hardcopies of the completed bid forms must be delivered to the District Office (5050 Madison Street, Skokie, IL 60077). 3. Bid forms may be hand-delivered by bidders or delivered via Fed Ex, UPS, USPS, or other delivery service. The District Office is open Monday-Friday, 8:00 a.m. – 4:00 p.m. CST. Bids may be delivered prior to Friday, November 20, during normal office hours. 4. Bids received after 11:00 a.m. CST will not be accepted, and the District is not responsible for delays in delivery through third-party carriers, such as Fed Ex and UPS. 5. Bids must be delivered in a sealed envelope that is clearly marked with the bid number. 6. Bids will be opened publicly at 11:00 a.m. CST on November 20, 2020, at the District Office. The bid opening will not be streamed online. Bidders must acknowledge receipt and acceptance of the bid addendum by signing below. Bidders must attach the signed addendum to the bid document prior to submitting the bid to the District.
