[#1] Request for Proposal (RFP): [2026-01-05 Architect] Issue Date: [5/1/2026] Due Date: [5/27/2026] This Addendum is issued to modify the above-referenced RFP and shall become part of the Contract Documents. Purpose of Addendum The RFP is hereby amended to require submission of a completed and signed NonCollusion Affidavit as part of each proposer’s response. Added Requirement All proposers must include a duly executed Non-Collusion Affidavit with their proposal submission. Failure to provide the affidavit may result in the proposal being deemed nonresponsive. The affidavit shall certify that: • The proposal was developed independently; • No collusion, consultation, communication, or agreement has occurred with any competitor regarding pricing or terms; • No attempt has been made to restrict competition; and • The proposer is authorized to execute the affidavit on behalf of the firm. Submission Instructions The completed Non-Collusion Affidavit must be included with the proposal package submitted by the proposal due date stated in the RFP. All other terms and conditions of the RFP remain unchanged.
