Original proposal plus six copies with original signatures (The proposal must be signed by an official authorized to legally bind the vendor to the information provided). One (1) electronic copy on a USB flash drive in MS Word or PDF format. 2. Must be currently registered with The Alabama Department of Finance, Division of Purchasing as a State Vendor and provide vendor number: Alabama Department of Finance-Purchasing Division 3. The vendor must complete the affidavit for business entity/employer/vendor. Verification of enrollment in E- Verify should be presented on the form found in Appendix A.
