This program shall provide mental health consultation services, short-term, high-level clinical care coordination/management services, individual and group therapy sessions, psycho-education and/or evidence based/informed modalities, engagement and linkage to resources and other services to older adults to help improve their mental health and wellbeing. These services shall enable the older adults to continue living and functioning in their own homes/communities thereby potentially saving costly emergency care due to inaction.
