Flagler County Schools is requesting proposals from qualified firms to offer medical services to employees, retirees, eligible spouses and enrolled dependents covered under the District's medical plan to include, but not limited to, primary care, preventive screenings, health risk assessments, acute and urgent care, immunizations, injections, disease management, and primary care case management through the utilization of a current medical facility, practice, clinic, urgent care center, etc., centrally located in Flagler County, FL.
