This RFP is the process by which the County will solicit proposals for the purpose of entering into a contract with selected respondents for a 12-month period from July 1, 2024 through June 30, 2025, with the possibility of an annual contract renewal and/or extension for three (3) additional years based upon satisfactory performance and available funding. The County of Solano, Department of Health and Social Services, Napa/Solano Area Agency on Aging (N/S AAA) located at 275 Beck Ave. Fairfield, CA 94533, hereinafter referred to as the “County,” intends to secure contracts with qualified proposers to provide Supportive Services, Family Caregiver Services, and Health Promotion Services to older adults and adults with disabilities.
