Through this RFA, the Heart Disease and Stroke Prevention Program will award up to 10 community pharmacies contracts to provide Medication Therapy Management (MTM) services to the defined patient population and participate in the Mississippi Alliance for Cardiovascular Health (MACH) Learning Collaborative (LC). The goal of this RFA is to partner with community pharmacies that can deliver comprehensive MTM services to enhance medication adherence, enhance therapeutic outcomes, and improve overall patient health, particularly for those with and at the highest risk of hypertension and high cholesterol. Preference will be given to community pharmacies that serve patients with or at risk of having cardiovascular disease in counties with limited access to health care and social service needs. Up to 10 eligible community pharmacies will receive $10,000 annually over the next three years, based on performance to implement the MACH Community Pharmacy MTM Program in one of the 26 Priority Counties. The priority counties are Amite, Calhoun, Claiborne, Clarke, Covington, Franklin, George, Greene, Grenada, Harrison, Jefferson, Jefferson-Davis, Lauderdale, Lawrence, Leake, Montgomery, Neshoba, Noxubee, Prentiss, Pike, Scott, Tippah, Tishomingo, Webster, Wilkinson, Yalobusha
