Description

The County of Sacramento, Department of Child, Family and Adult Services (DCFAS), is seeking a qualifying organization or individual to develop a Local Aging and Disability-Friendly Action Plan (LADAP) for Sacramento County in accordance with the California Department of Aging (CDA) LADAP Grant Program and the AARP Age-Friendly Network of States and Communities. Up to $200,000 is available for a possible 21-month period that may begin July 1, 2023, and must end by March 31, 2025. Services and activities associated with ensuring a successful outcome include, but are not limited to, the applicant's ability to complete a comprehensive LADAP and related activities: Convene and facilitate at least ten age and disability-friendly outreach and action plan listening sessions or focus groups with older adults, caregivers, and people with disabilities from underserved and under-represented populations, businesses, and arts and cultural groups. Seventy percent of the sessions must be in-person and located at accessible locations primarily in unincorporated areas of the County. The sessions or focus groups must be completed no later than September 2024.Collect, research, track, and analyze data specific to the LADAP planning grant.Submit progress reports as requested by DCFAS or CDA. Complete a final, comprehensive public action plan that fulfills the requirements of the CDA local plan and the AARP action plan.Conduct a marketing and awareness campaign and organize and lead an in-pe
