Lift Station Rehabilitation – LS-04 N.20th Street located at 2001 N. Daytona Avenue PS-08 Lantana Avenue located at 700 Moody Lane Prepared by: City of Flagler Beach PROJECT OVERVIEW The City of Flagler Beach is rehabilitating two wastewater lift stations located at N. 20th Street and at the Lantana Avenue. The project includes demolition of existing internal components, structural repairs, installation of new pump bases, replacement of interior piping, installation of new valves, fabrication and installation of new access covers, and associated concrete modifications. The contractor is responsible for all labor, materials, equipment, permitting, wet well cleaning, safety compliance, and installation required to complete the work. This SOW also establishes mandatory proposal submission requirements that contractors must follow when submitting pricing, schedules, and documentation. Contractors shall attend the mandatory pre bid meeting and site visit to LS 04 and PS 08. Attendance will be documented and is required for bid responsiveness. LOCATION & ACCESS REQUIREMENTS N 20th Street Lift Station •    Confined space entry required. •    Contractor shall fully clean the wet well prior to beginning any demolition or installation work. •    Contractor is responsible for obtaining all required permits, including confined space, ROW, utility, and any regulatory permits required for the work. •    Contractor shall coordinate access with City Utilities staff. Lantana Avenue Under Bridge •    Confined space entry required. •    Contractor shall fully clean the wet well prior to beginning any demolition or installation work. •    Contractor is responsible for obtaining all required permits, including confined space, ROW, utility, and any regulatory permits required for the work. •    If force main isolation is not possible, contractor must identify and permit any additional measures required to complete tie ins.
