The goal of this RFP is to fund community based activities to reduce risks for poor pregnancy and birth outcomes, including premature births, low birth weight births, and infant death for the African American community in Sacramento County. Trained peer mentors will provide education, support, accompaniment, and navigation services to pregnant African American people. A maximum of $2,520,000 is available to implement the program from July 1, 2024 - June 30, 2027. Government agencies are not eligible to apply.
