Employee Health Clinic Services

Description

Contractor shall provide Employee Health Clinic Services for City Eligible Employees and their Eligible Dependents covered on the City’s Active medical insurance plans.Contractor shall establish and operate the near site clinic, at its own cost and expense, which shall include (a) leasing or owning the real property on which the near site clinic will operate, (b) constructing or leasing the building, (c) furnishing the near site clinic, including the design, equipment, signs, interior and exterior decor items, fixtures, furnishings, supplies and other products and materials, and (d) any costs for any future renovation or remodeling of the property.Contractor shall provide same or next day medical appointments and on-site pharmacy services.
