In order to gain economies of scale, Mohave is formally soliciting statewide sources of E-Rate internal connections and basic maintenance as specified within this Request for Proposal. These products/services are requested for Mohave’s membership of over 480 public agencies. This contract is written to accommodate our members’ needs for E-Rate compliant pricing for our member’s FCC Form 471 process and to provide FCC Form 470 compliant procurement that can be referenced as the establishing Form 470 on the E-Rate application. Contracts, in whole or in part, shall be awarded to offeror(s) for a full term, not to exceed a maximum of 60-months total. This contract term is for the benefit of Mohave’s members in regard to Universal Service Administrative Company’s (USAC) funding cycle.Mohave’s Form 470 Application Number is: 240002243 Initial activity under the contract shall be for E-Rate funding year 7/01/2024 – 6/30/2025.
