In the lower left corner of the envelope, print your company name, Dish Machine Bid, and the time and date of opening. All bids will be opened publicly. Vendors will receive official notification of the bid award, pending Board approval. Bids must be received prior to the designated time for bid opening. Bids received after the designated time of opening will be considered late and will not be accepted. Ship and Deliver – All equipment and supplies for the items bid are to be delivered to school as directed by the purchase order. All price quotes must be F.O.B. destination. In addition, the equipment is to be installed and operational as part of this bid requirement. Packaging material, including cardboard and pallets, are to be removed from the premises. 2. Quality and Guarantee – All materials on which bids are submitted shall be of quality and grade specified. Each bid must be accompanied by complete descriptions, catalog cuts, or other illustrations of each item upon which a bid is made, if appropriate. The names of manufacturers, model numbers and stock numbers shall be clearly indicated. 3. Requirements for Submitting Bids – Bids made on forms other than the Proposal Form may not be considered.
