The purpose of this request for proposal (RFP) is to identify and develop a list of agencies that have interest in providing corporate foster care services for Sherburne County waiver recipients that qualify for an exception to the corporate foster care moratorium. Individuals that qualify for corporate foster care moratorium exceptions typically have high behavioral and/or medical needs and cannot be served within Sherburne County’s existing capacity. Providers will partner with Sherburne County Disability Services and the program will serve waiver recipients.
