CalAIM Justice-Involved Pre-Release Medi-Cal Application Outreach

Description

The County of Santa Cruz invites sealed proposals from fully licensed, insured, bonded, and certified vendors to furnish all labor, tools, equipment, and incidentals required to provide Project Title. This support is particularly focused on Justice-Involved (JI) individuals who are released from incarceration quickly, are housed in juvenile justice facilities, or require additional assistance with their applications. This contract is funded through the California Department of Health Care Services (DHCS) under the California Advancing and Innovating Medi-Cal (CalAIM) Providing Access and Transformation (PATH) Justice-Involved Capacity Building Round 2 initiative. It will establish a joint agreement between the selected organization and the County of Santa Cruz Human Services Department (HSD), the Probation Department (PRB), and the Sheriff’s Office (SHF) with the goal of enhancing coordination and improving access to essential services for individuals transitioning out of jail or juvenile hall.
