Take careful note of the addendum so that proper allowance may be made. 1. Change on Information for Bidders, page 3, item 16, Delivery Requirement: Should read: Bidders shall bid all items F.O.B. destination, FRT. Prepaid to: GGUSD Warehouse, 10331 Stanford Ave., California, 92840 and reference the purchase order number that will be given when an order is placed. Vendor will require to contact an identified District staff member at least two days prior to delivery. District warehouse has a loading dock, pallet jack and/or forklift. Inside delivery is not required. 2. Change on Information for Bidders, page 6, Item 33, Drug and Alcohol Free Workplace: Should read: The successful bidder hereby certifies, under penalty of perjury, under the laws of the State of California that under the contract he will comply with the requirements of the Drug-Free Workplace Act of 1988 (Government Code Section 8350 et. seq.). 3. Change on Information for Bidders, page 6, item 34, Tobacco-Free Workplace: Should read: In the interest of public health, the Garden Grove Unified School District provides a tobacco-free environment. Smoking or the use of any tobacco products are prohibited in buildings and vehicles, and on any property owned, leased or contracted for, by the District. Failure to abide with this requirement could result in the termination of the contract. 4. Addition to Information for Bidders, page 7, item 39, Term of Contract: The term of this contract is for one (1) year with an option to extend for three (3) years, one (1) year at a time.
