RFP No. 2026-0064-PW/DK SANTA FE COUNTY ATTACHMENT G – REVISED COST PROPOSAL FORM RFP No. 2026-0064-PW/DK Water Reclamation Facility Expansion - Design-Build Project Phase II - Volume II Cost Proposal 1. Instructions  Submit this completed Cost Proposal Form in Volume II - Cost Proposal only. Do not include pricing in Volume I.  Submit one complete Lump Sum Cost Proposal and one corresponding General Schedule of Values. The lump sum must equal the total cost of the design and construction solution presented in Volume I.  Select whether the proposed solution incorporates no ATCs or incorporates one or more ATCs. If ATCs are incorporated, identify each one in Section 3 by the number used during the confidential ATC process and provide a very brief description.  Only ATCs presented during the confidential ATC process and receiving a verbal 'Yes' response may be incorporated. A verbal 'Yes' authorized inclusion in the proposal but did not constitute final acceptance of the ATC or award of a contract.  The Offeror's technical proposal, Lump Sum Cost Proposal, and General Schedule of Values must all be based on the same proposed design and construction solution, including all identified ATCs.  The Lump Sum Cost Proposal will be used as the Offeror Price in the RFP cost-scoring formula. No separate ATC prices, alternates, additions, or deductions will be evaluated. Pricing basis: All prices stated on this form exclude New Mexico Gross Receipts Tax (NMGRT). 2.
