The County of Sacramento, Department of Child, Family and Adult Services (DCFAS), is seeking a qualifying organization or individual to develop a Local Aging and Disability-Friendly Action Plan (LADAP) for Sacramento County in accordance with the California Department of Aging (CDA) LADAP Grant Program and the AARP Age-Friendly Network of States and Communities. Up to $200,000 is available for a period beginning upon contract execution and ending by March 31, 2025. Services and activities associated with ensuring a successful outcome include, but are not limited to, the applicant's ability to complete a comprehensive LADAP and related activities: Convene and facilitate at least ten age and disability-friendly outreach and action plan listening sessions or focus groups with older adults, caregivers, and people with disabilities from underserved and under-represented populations, businesses, and arts and cultural groups. Seventy percent of the sessions must be in-person and located at accessible locations primarily in unincorporated areas of the County. The sessions or focus groups must be completed no later than August 2024.Collect, research, track, and analyze data specific to the LADAP planning grant.Submit progress reports as requested by DCFAS or CDA.
