Town of Surfside RFQ 2023-02 Design Services for the Surfside Memorial Addendum No. 5 Date Issued: July 6, 2023 To All Respondents: Respondents for the above-referenced RFQ shall take note of the following changes, additions, deletions or clarifications to RFQ No. 2023-02, which in accordance with the RFQ Documents shall become a part of and have precedence over anything shown or described otherwise in the RFQ. TOWN RESPONSES TO QUESTIONS RECEIVED JULY 5th, 2023, ARE PROVIDED BELOW: 1. Since yesterday was a holiday and the submittal deadline is on August 3rd, would it be possible to extend the question deadline for another week? Please take note the deadline to submit questions is extended until Friday, July 7th, 2023, at 5:00 P.M. (EST). 2. Do we need to include pricing as part of our proposal? No, pricing is not part of the proposal as this is an RFQ. 3. Is additional funding available for design teams? This question is not clear. Please note funding for this project is coming from multiple funding sources and the Town is still seeking additional funds. 4. The Town’s February 28th meeting notes indicate that survey for 88th Street is underway- will this be provided/ available ahead of the RFQ response deadline? Response will come in a later addendum. 5. Will the Town of Surfside assist in expediting AHJ approvals required for the design including but not limited to FDOT/ FDEP? 1 Response will come in a later addendum. 6. As requested in section D. Qualifications/Experience of the Firm/ Respondent, Item c. -- Are the business licenses, occupational licenses Florida registration, and State Corporate Certificate included in the 20-page limit? No, this is excluded from the 20-page limit and from the font size requirements. 7. As requested in section D. Qualifications/Experience of the Firm/ Respondent, Item d. -- Are the professional and business licenses included in the 20-page limit?
