Town of Surfside RFQ 2022-04 Construction of Town of Surfside 96th Street Park Addendum No. 2 Date Issued: June 20, 2022 To All Proposers: Proposers for the above-referenced RFP shall take note of the following changes, additions, deletions or clarifications to RFP No. 2022-04, which in accordance with the RFQ Documents shall become a part of and have precedence over anything shown or described otherwise in the RFP. THE FOLLOWING CHANGES ARE MADE TO THE RFP: Are you able to provide any details/specs regarding the outdoor fitness equipment for this project? 1. The drawing sheet LF-01 included in the bid drawings posted with the RFQ identifies the specified equipment. 2. Can you clarify the specified playground specification where it referenced Berliner Combination 1 and 2. What design line is specified from Berliner? Combination 1 & 2 are custom pieces designed by Berliner 3. For the Phase 2 Evaluation of Price, which is worth 80 points. Is it correct to assume that the firm with the lowest price will be awarded the full 80 points? If so, how will points be awarded to the other higher firms? Is there a formula that will be used? If so, can you please share that formula. It is the discretion of the evaluator, but it will be tiered with lowest bidder receiving most points and highest bidder receiving least points. 4. Please confirm that the Performance and Payment Bonds are due by the awarded firm only after award and are not due at the time of the initial submittal. The Town will revise Bid Package requirements to only require a Bond Form along with Price Submittal Form in Price Submittal Package. A Payment and Performance bond will be required to be produced by the awarded firm within 10 days from notice of award by the Town. 5. For the Price Submittal Package and Bond Package which is to be included in a separate sealed enclosure, please confirm how many copies of this package are required. One copy Sealed 6. Tab 3. Proposer’s Qualifications includes the requirement to list the tasks, if any, that will be subcontracted out and provide a summary of the experience and qualifications of the lead personnel for each sub-contractor that will oversee performing this work. Can you please confirm if the Town is referring to subconsultants that may be part of our team or subcontractors that bid the project as it is unusual to provide this information for subcontractors. This is the discretion of the bidder. The evaluation will be based on submitted documents.
