Posted: October 2, 2026 ADDENDUM NO. 1 REQUEST FOR QUALIFICATIONS (RFQ #2026-005) ENGINEERING SERVICES CITY OF MERCEDES, TEXAS TO ALL PROSPECTIVE PROPONENTS: This Addendum No. 1 forms a part of the RFQ #2026-003 (Engineering Services) and modifies, clarifies, and/or supplements the original RFQ specifications posted on September 10, 2026. Proponents shall acknowledge receipt of this Addendum No. 1 in their proposal submittal. RESPONSES TO VENDOR QUESTIONS: The following subsection hereby clarifies questions to the RFQ specifications: 1. Clarification of how many originals (bound or unbound) and electronic copies need to be submitted for the RFQ. The City is requesting three (3) bound copies and one (1) electronic copy on a USB in PDF format. 2. Forms in Section 15 – Required Forms and Certifications. Does the City intend to provide these forms? Yes. Attached to this Addendum are the official forms required for submission: a. Conflict of Interest Questionnaire (Form CIQ) b. Non-Collusion Affidavit c. Debarment & Suspension Certification d. IRS Form W-9 e. New Vendor Setup Packet (Purchasing Department) f. Combined Acknowledgements Form (Attached; covers Public Information Act Acknowledgement and Insurance Requirements Acknowledgement) Note: No separate form is required for Statutory Certifications. Bidders are expected to comply with all applicable local, state, and federal laws and statutory regulations in their standard operations and submittals. 3. Clarification on the Placement of the required forms. Required form will be placed at the end of your proposal. 4. Placement of Exhibits A – E: Where does the City want Exhibits A-E to be placed in the proposal? Would they like to see them in the sections that they align with? Yes, please align the exhibits with the section they pertain to. (For example: Exhibit C would go in Section 9 with the Availability and Responsiveness information.) END OF ADDENDUM NO. 1 FORM CIQ CONFLICT OF INTEREST QUESTIONNAIRE For vendor doing business with local governmental entity This questionnaire reflects changes made to the law by H.B. 23, 84th Leg., Regular Session. This questionnaire is being filed in accordance with Chapter 176, Local Government Code, by a vendor who has a business relationship as defined by Section 176.001(1-a) with a local governmental entity and the vendor meets requirements under Section 176.006(a).
