EXTENSION PROJECTS ADDENDUM NO. 1 Dear Proponents, This addendum is issued to reflect the following immediately. It shall be the responsibility of interested Bidders to adhere to any changes or revisions to the IFB as identified in this Addendum No. 1. This documentation shall become permanent and made part of the departmental files. Attachment A: Pre-Proposal Agenda Attachment B: Updated Bid Form Question No. 1: Do the non-collusion affidavit for subcontractor and the certification of subcontractor regarding equal employment opportunity need to be turned in with the bid, or are these to be provided by the subcontractor after award? Answer No. 1: Only the prime bidder’s Non-collusion Affidavit and Equal Employment opportunity forms are required to be included in the bid. Once the contract is awarded, the subcontractor’s documents will then be required. Question No. 2: On Avenida del Sur, the Utility sheets and cross sections show the 4” natural gas line in the Base Course, AC Course, and above the final roadway surface. Will the 4” natural gas line be relocated prior to NTP and if so, where will it be relocated to? Answer No. 2: The Gas line will be relocated to the south side of the roadway prior to the estimated start of the phase 2 construction of ADS. It will be relocated between the ROW and the water line in the area where there is cut in the roadway, following the relocation of the overhead electric poles, which is expected to begin in September. 102 Grant Avenue · P.O. Is that additional ROW being acquired? If so, where will the new ROW limits be relocated? Answer No. 3: The only location where the sidewalk is currently shown outside of the right of way is at the western edge (STA 23+00). This area is currently in the acquisition process and will include the triangle area up to the culvert outflow. Question No.
