Department of Human Services Homeless Assistance Program #052526-HAP Request for Proposals The Washington County Department of Human Services’ mission is to create an accessible and holistic integrated Human Services System that addresses the individual and family needs of Washington County to promote self-sufficiency and improve their quality of life. Our vision is to provide the residents with the highest quality of health and human services throughout their lifespan. 2026-2027 Department of Human Services Homeless Assistance Program The Homeless Assistance Program (HAP) is designed to mitigate the impact of homelessness on individuals and families through the provision of housing supports, prevention services, and comprehensive case management. The program prioritizes both immediate stabilization and long-term housing security. The primary objective of HAP is to assist individuals and families experiencing homelessness, or at imminent risk of homelessness, in achieving self-sufficiency and securing permanent housing. Services are structured to support participants in obtaining and maintaining stable living arrangements, with permanent housing as the ultimate outcome. Eligible Program Components Case Management This service assists participants in identifying housing barriers and the underlying causes of homelessness or risk of homelessness. Case Management focuses on equipping participants with the tools and skills necessary to achieve long-term housing stability and self-sufficiency. Services may include budgeting, life skills development, employment preparation, housing stability planning, and referrals to supportive services such as substance use treatment, as appropriate. Rental Assistance Rental Assistance provides financial support to households at risk of eviction or housing instability. Eligible assistance may include payments for rent, mortgage arrears, security deposits, and utilities. Providers will work collaboratively with landlords and participants to promote housing retention or to secure safe, affordable alternative housing. Assistance may also support transitions from shelter into permanent housing. Bridge Housing Bridge Housing serves as a transitional step between homelessness and permanent housing. This component offers temporary housing in shared or individual units for up to 18 months, with participant contributions based on income.
