ADDENDUM #3 RFP 25-008 2025 LMIG Street Paving This addendum is being issued to address questions submitted regarding RFP 25-008: 1. The RFP indicates we are to submit with our proposals a “Vendor Master Form” (page 4 , item 1.12). Such a form is not among the forms included in the RFP. Please provide this form if it is required. See attached. 2. On page 5 of the RFP, item 1.17.1, it is noted that we are to submit “…and any Technical Requirements (in Excel format)”. What sort of technical requirements would this be? Technical Requirements (in Excel format)" for a road paving project, would typically include details about the specific machinery, materials, and quality standards needed for the paving process, such as: pavement thickness, aggregate type, asphalt grade, compaction specifications, milling depth, equipment capabilities (rollers, pavers), testing methods, environmental considerations, and any necessary permits or compliance requirements; all organized in a structured format within an Excel spreadsheet for easy reference and analysis This addendum should be signed and returned with your proposal. Failure to do so may result in the proposal being non-responsive.
